2011-12 Physicals

 

 

 

All athletes MUST have a current OHSAA pre-participation physical exam and HIPAA Form on file in the Athletic Office prior to tryouts.  Athletes must take a copy of the OHSAA pre-participation physical form with them at the time of the physical.

 

The Athletic Department urges all Seton athletes to contact their primary care physicians to set up an appointment for their pre-participation physicals.  Your primary care physician provides year-round comprehensive health care, and he or she will provide any vaccinations that are required, review your health history and counsel you about safety in your sport.

 

Wellington Orthopaedic & Sports Medicine offers pre-participation physicals for the 2011-2012 school year.  Physical exams cost $10 and each student athlete will receive a t-shirt when the physical ends.  If you have any questions, please contact Lisa Sweeterman at (513) 245-5434.
