All athletes MUST have a current OHSAA Pre-participation Physical Exam and HIPAA Form on file in the Athletic Office prior to tryouts. Athletes must take a copy of the OHSAA pre-participation form with them at the time of the physical.

Seton High School Athletic Department urges all Seton athletes to contact their PRIMARY CARE PHYSICIAN to set up an appointment for the pre-participation physicals. Your primary care physician provides year round comprehensive health care. They also will provide any vaccinations that are required, review the athlete’s health history, and provide counseling regarding safety.
Wellington Orthopaedic & Sports Medicine also offers pre-participation physicals for the 2010-2011 school year. The cost for the physical exam will be $10, and all athletes receive a t-shirt.
