2008-2009 Physicals

All Athletes MUST have a current OHSAA Preparticipation Physical Examination and HIPAA Form on file in the Athletic Office prior to tryouts. Athletes must take a copy of the OHSAA preparticipation form with them at the time of the physical.
Athletes may pick up a copy of the OHSAA preparticipation and HIPAA form from the main office or athletic office. A copy of the OHSAA preparticipation and HIPAA form is below and may be downloaded.

OHSAA Pre-Participation Physical and HIPAA Forms

Seton High School Athletic Department urges all Seton athletes to contact their PRIMARY CARE PHYSICIAN to set up an appointment for the pre-participation physicals.

Your primary care physician provides year round comprehensive health care. They also will provide any vaccinations that are required, review the athletes health history, and provide counseling regarding safety.

Wellington Orthopaedic & Sports Medicine also offers pre-participation physicals for the 2008-2009 school year. The cost for the physical exam will be $10, and all athletes will also be given a t-shirt. See below for dates, times and forms for Wellingtion physicals.
